1. USAGE; COPYRIGHT: Any promotional items with the SOUL & SOLE Community Organization LLC logo on them, including but not restricted to flyers, business cards, t-shirts, water bottles, etc. contain material which is owned by us. You may not reproduce, copy, or redistribute the design or layout of the SOUL & SOLE Community Organization LLC format. All rights reserved.
2. CANCELLATIONS: Since there is a limited amount of space in camp (50 Players Maximum), for cancellations, please notify us via telephone or email by July 24, 2018.
3. WEATHER: SOUL & SOLE Community Organization shall not be accountable should activities be suspended or cancelled due to adverse weather. In cases where the weather has affected the condition of the playing surface or is deemed so bad that activities have to be suspended, the staff will seek to supervise alternative activities, but it is not guaranteed.
4. USAGE (2). Photos and videos will be taken during activities for business marketing purposes.I HAVE READ AND AGREE TO THE TERMS AND CONDITIONS BY ELECTRONICALLY SIGNING BELOW.

ATHLETIC WAIVER AND RELEASE OF LIABILITY: In consideration of being allowed to participate in training provided by SOUL & SOLE Community Organization LLC, the undersigned acknowledges, appreciates, and agrees that: 1. The risk of injury from the activities involved in soccer is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation and/or my child’s participation. 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself or my child/children from participation and bring such to the attention of the nearest health official immediately. 4. I, on behalf of my child/children participating, HEREBY RELEASE AND HOLD HARMLESS SOUL & SOLE Community Organization LLC and its staff, venue hosts and owners of the company WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY ENTERING MY NAME BELOW.
