Terms and Conditions 1. USAGE; COPYRIGHT: Any promotional items with the SOUL & SOLE Community Organization LLC logo on them, including but not restricted to flyers, business cards, t-shirts, water bottles, etc. contain material which is owned by us. You may not reproduce, copy, or redistribute the design or layout of the SOUL & SOLE Community Organization LLC. All rights reserved. 2. REFUNDS: Please refer to training guidelines for refunds. If player(s) cancels less than 6 hours before scheduled session time, there will be no refund. 4 Session packages must be used within 6 weeks of purchase. Semi-private sessions are priced per person. Sessions will not be extended if player is late. 3. WEATHER: SOUL & SOLE Community Organization shall not be accountable should activities be suspended or cancelled due to adverse weather. In cases where the weather has affected the condition of the playing surface or is deemed so bad that activities have to be suspended, the staff will seek to supervise alternative activities, but it is not guaranteed. 4. Liability Waiver ATHLETIC WAIVER AND RELEASE OF LIABILITY: In consideration of being allowed to participate in training provided by SOUL & SOLE Community Organization LLC, the undersigned acknowledges, appreciates, and agrees that: 1. The risk of injury from the activities involved in soccer is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation and/or my child’s participation. 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself or my child/children from participation and bring such to the attention of the nearest health official immediately. 4. I, on behalf of my child/children participating, HEREBY RELEASE AND HOLD HARMLESS SOUL & SOLE Community Organization LLC and its staff, venue hosts and owners of the company WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY CHECKING THE BOX BELOW.

Covid -19 Waiver *

Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and show no symptoms and therefore spread the disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death. SOUL&SOLE Community Organization LLC cannot prevent you [or your child(ren)] from becoming exposed to, contracting, or spreading COVID-19 while utilizing SOUL&SOLE Community Organization LLC’s services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize [name of company]’s services and/or enter onto SOUL&SOLE Community Organization LLC’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19. ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my children in order to utilize [name of company]’s services and enter [name of company]’s premises. These services are of such value to me [and/or to my children,] that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize SOUL&SOLE Community Organization LLC’s services and premises in person [if applicable: “rather than arranging for an alternative method of enjoying the same services virtually (e.g. videoconference)]. WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against [name of company] and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing SOUL&SOLE Community Organization LLC’s services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen. CHOICE OF LAW: I understand and agree that the law of the State of Florida will apply to this contract. I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE. I am the parent or legal guardian of the minor named above (If under the age of 18).
